Connect Care Choice
Connect Care Choice is a health care option for adults who have Medical Assistance coverage and are 21 year old or older. The goal of Connect Care Choice is to improve access to primary care, help coordinate health care needs, and link to support services in the community.
To be eligible for Connect Care Choice you must be eligible for Medical Assistance and
- a Rhode Island resident
- 21 years old or older
- live in the community (at home, in assisted living, or a group home)
- cannot receive Medicare or other health insurance
Adults who become eligible for RI Medical Assistance, will receive a letter from the Department of Human Services offering them a choice of health care options-either enrollment in a Connect Care Choice primary care practice or in a participating Rhody Health Partners health plan. Both Rhody Health Partners or Connect Care Choice offer many locations for Medical Assistance recipients.
