Steps in the Application Process- How to Apply
Complete the application as best as you can. If you need help completing the application, call one of the numbers below.
Documents you MAY NEED to include with your application: Identification (driver's license, birth certificate) Social Security numbers for everyone that is applying. (You don't have to have a Social Security card.) Proof of the past month's income (earned, unearned, self- employment, rental income, etc.) Proof of the past month's housing and utility expenses such as mortgage statements, rental agreements, and bills Proof of expenses such as day care and child support payments Proof of medical expenses for people 60 or older or for individuals with disabilities Proof of assets such as bank accounts, stocks, and investments Documentation of citizenship status
To determine eligibility, you may need to bring these documents (for all household members) when you go to the interview. Photocopies of documents will be accepted.
You will be required to have an interview with a DHS worker. He or she will go over the application with you and make sure you have completed everything that is required. You can choose to do this by phone or in person. If you choose by phone, your interview will be scheduled after the DHS worker receives your application in the office.
Once we receive a completed application and you have participated in your intake interview, we will notify you to let you know if you qualify. If you qualify for SNAP benefits, you will receive an EBT card (Electronic Benefit Transfer) which is similar to an ATM card. You will use this at the supermarket to buy food.
