This Provider Search has been created to assist in locating Medical Assistance doctors, specialist, therapist, and other supporting services. This list does not guarantee the provider is currently accepting new Medicaid clients. If you are a Managed Care client, please contact your health plan for participating providers.

To use this search, select the type of service from the drop down menu and/or enter criteria in the fields below and then click on the Find Provider button.

Use the Reset Form button to clear the form between each search.

For help on a particular field, click on the field name.

Enter data into at least one field other than State.
Enter search criteria into the desired fields and click "Find Provider".
Click the Reset Form button to return form to its default values.

Type of Service

Select a provider type of service from the pull-down list..
Leave field blank when you do not wish to search by provider type.

Institution

Enter the institution's name into the field.
Leave field blank when you do not wish to search by institution name.
Leave field blank when searching by last name.

Last Name

Enter the last name into the field.
Leave field blank when searching by institution.
Leave field blank when you do not wish to search by last name.
The last name is required when the first name field is entered.

First Name

Enter the first name into the field.
Leave field blank when you do not wish to search by first name.

City

Enter the city into the field.
Leave field blank when you do not wish to search in a specific city.

State

Select the appropriate State abbreviation from the list.
The default value of this field is "RI".
To search by all states, select the blank row at the top of the list.

Zip code

Enter a 5 digit zip code.
Leave field blank when you do not wish to search in a specific zip code.

Sort by

Select the desired sort option for the provider list that will be returned.
