Long Term Care
Program Description
Long Term Care services consist of home and community based services and institutional
care. The intent of the Long Term Care program is to provide the most appropriate service
in the least restrictive setting. All applicants are assessed to determine their medical
and functional status and only those determined to need an institution or nursing facility
level of care are eligible. Home and community based services are an alternative to
institutional care or nursing home care. See below for more information.
Who Can Qualify?
- Individuals over 65 years of age
- Individuals with disabilities
To be eligible, individuals must meet certain financial and program criteria and be
determined to require the level of care provided in an institution or nursing facility.
Eligible individuals are offered a choice between an institution/nursing facility or home
and community based services.
Description of Services
Institutional Services
Institutional Services provide 24-hour care to individuals with disabilities or the
elderly with significant levels of impairment. Residents receive room and board,
supervision, nursing services, transportation, recreational and social services. Necessary
medical services not included in the daily rate are arranged for as needed. Medical
Assistance recipients can receive any of the services included in the regular Medical
Assistance scope of services to the extent that they do not duplicate services included in
the facilities per diem rate.
Home and Community Based Services
Home and Community Based Services are available with the intent that many individuals
can be cared for in their homes and communities thereby preserving dignity and
independence. A variety of services may be available including: homemaker services, case
management services, home health aide services, personal care services, respite care
services, minor home modifications, residential assisted living, and habilitative
services. These services are called "waiver programs" and not covered under the
States Medicaid plan.
