Program Description

Connect Care Choice is a new health care option for adults who are 21 years old or older with Medical Assistance coverage. Eligible adults are offered an option to participate in Connect Care Choice at their physician’s office. The goal is to improve a patient’s access to their primary care doctor and also to get help coordinating their health care needs and support services in the community.
 

Who's Eligible

Adults who live in Rhode Island and –

  • Are 21 years old or older,

  • Have Medical Assistance coverage (not Medicare or other health insurance),

  • Live in the community (at home, in assisted living, or a in group home).
     

How to Apply

Eligible adults will receive an enrollment letter from the Department of Human Services (DHS) offering them a choice to participate in Connect Care Choice. Participation is voluntary. See below for a list of participating Connect Care Choice practices.
