Program Description Connect Care Choice is a new
health care option for adults who are 21 years old or older with Medical
Assistance coverage. Eligible adults are offered an option to participate in
Connect Care Choice at their physician’s office. The goal is to improve a
patient’s access to their primary care doctor and also to get help

enrollment letter from the Department of Human Services (DHS) offering them
a choice to participate in Connect Care Choice. Participation is voluntary.
